The nondiscrimination notice names Ruby Reyna as the grievance contact at the Garland Road address. Its number and email have a specific civil-rights purpose. For a routine scheduling, statement or refund question, start with the treating office or general patient contact instead of assuming that every concern belongs with the compliance coordinator.

The older 214-321-8400 headquarters and HR label, and the separate 866-924-8212 complaint-line listing, were not reconfirmed in the current official pages reviewed. They are not presented here as verified company-wide contacts. Likewise, the Garland Road correspondence address does not establish that it is the correct destination for every patient’s records or payment.

How to Escalate a Smiley Dental Service or Billing Concern

Start with the practice that provided the service. An office manager can help identify the account and the person responsible for responding. For a question about care, request the treating dental professional or appropriate clinical team. The following steps are suggestions for preparing an organized inquiry:

  1. Identify the office and visit. Have the location name, appointment date and provider’s name available. If you visited more than one Smiley office, explain which visit produced the charge or concern. Supply patient identifiers privately when the office asks for them.
  2. Describe the issue in specific terms. Explain whether the question involves a scheduling change, office interaction, estimate, insurance submission, payment or treatment follow-up. State what you were told and what remains unclear without guessing about another person’s motives.
  3. Gather supporting records. Keep the treatment estimate, itemized statement, payment receipt, appointment confirmation and relevant correspondence. For an insurance question, have the explanation of benefits available as well. Organize the dates so the office can compare the records.
  4. Ask for the appropriate review. Request an explanation of the disputed item or a manager’s review of the service concern. If further company assistance is needed, ask the office who handles it and how to submit your information securely. Retain any reference number.
  5. Confirm the next step in writing. Ask what documents are still needed, which department will respond and how the response will arrive. Keep the reply with your account records and identify any unanswered question when following up.

For a refund request, distinguish the original payment from an insurance adjustment or a remaining account balance. Ask the office to explain the amount under review and the payment method associated with it. A request for review does not itself confirm that a refund has been approved, and this guide does not establish a universal processing time.

When writing about an office interaction, use a short timeline and identify the role of the person you spoke with. A focused description is easier to investigate than several unrelated concerns combined into one message. If another person is communicating for you, ask the practice what authorization it requires before discussing the account.

Patient Records, Insurance and Ongoing Treatment

For copies of records or a transfer to another dentist, contact the office holding the records and ask for its release procedure. Specify whether you need radiographs, treatment notes, an account statement or a combination of documents. Confirm the recipient, delivery method and any required authorization before sending personal information.

For an insurance question, compare the office’s statement with the explanation of benefits from the insurer. Ask the office which claim was submitted and whether additional information is outstanding. Contact the insurer for questions about its coverage determination or processing. These are different records, so keep both documents when requesting clarification.

Smiley’s patient-resources page describes payment options that include cash, credit cards, in-house financing and CareCredit, along with insurance information. Confirm the arrangements with the actual office before agreeing to treatment. Ask for a written estimate identifying the proposed services, estimated insurance contribution and expected patient amount; an estimate should not be treated as proof of final insurer payment.

If financing was used, identify the lender or account provider on the financing agreement. Questions about a dental-office charge and questions about a lender’s statement may require separate contacts. Keep any office adjustment or refund confirmation so you can ask how it will appear on the associated account.

For ongoing orthodontic treatment, a transfer or a missed follow-up, speak with the treating office about the next appointment and records needed. Explain whether another provider is involved and ask who will coordinate the handoff. Do not assume that a change of location automatically transfers a treatment agreement, scheduled visit or payment arrangement.

Smiley Dental Corporate Office Questions

Where is Smiley Dental’s corporate office?

The company’s privacy and nondiscrimination policies publish 10901 Garland Road, Dallas, Texas 75218 for correspondence. We did not independently verify a separate executive headquarters. Use that address with its published purpose and confirm the department before sending documents or arranging a visit.

Which number should I use for an appointment?

The current location pages publish 866-949-1639 as a general contact. Identify the city and office you want when calling. If you are changing an existing appointment, have the confirmation available and request confirmation of the new arrangements rather than assuming that an inquiry has changed the schedule.

Is the compliance email a general complaint address?

The company publishes compliance@smileydental.net in its nondiscrimination notice for civil-rights grievances. Use the notice to understand that process. Routine account, appointment and service concerns should begin with the relevant practice or general patient contact, which can identify the appropriate department.

How do I ask a question after dental treatment?

Contact the treating practice and explain that your question concerns a recent procedure or an ongoing treatment plan. Ask to speak with the appropriate clinical team. A corporate directory or public review cannot assess your condition or replace instructions from the dental professional responsible for your care.

What is Smiley Dental’s HR or payroll phone number?

A dedicated public HR or payroll telephone was not verified. Applicants should follow the careers page and specify the role and location. Employees should use the employer contacts supplied by their workplace; patient-service and policy numbers should not be relabeled as employee hotlines.

About Smiley Dental & Orthodontics

This guide concerns the Texas Smiley Dental & Orthodontics network associated with smileydental.net. Its location directory and patient information cover dental and orthodontic offices in several Texas communities. Because other practices can use similar names, confirm the website, street address and business named on your paperwork before sending an inquiry.

For other dental organizations, see our Aspen Dental and Monarch Dental corporate office guides.

Why Trust Corporate Office Headquarters?

We checked Smiley’s location directory, privacy policy, nondiscrimination notice, patient-resource page and careers information on September 12, 2026. Phone numbers and contact details were checked against published sources, not by calling. Corporate Office Headquarters is independent and is not affiliated with Smiley Dental & Orthodontics. Practical preparation and escalation steps are our suggestions; the company’s own procedures and your transaction documents determine how a request is handled.

Smiley Dental Customer Service Issue

April 29, 2024

bad experience waste of time! I suffer from Agoraphobia and haven’t been able to leave my neighborhood for over 20 years, I’ve had a loose wisdom tooth for a year So I got medicated to be able to get to the dentist to have an extraction. I had a bad tooth pain and infection. i got to my appt. on time and tried really hard to be able to stay in the office. I got x-rayed had an exam and waited for about an hour. When I finally saw the dentist she focused on my deep cleanings but not what I was there for they suggested for an oral surgeon to do the extraction. I have to wait about three weeks to see the surgeon about two hours later I got out of the examine room because I was left there. I got no appointment for a second visit or a receipt to show I was even there. I was very dissapointed because it took so much for me to get there. Came home in the same pain as when I got there. The sorry thing is This was the closest place I can go to.
